Report of Health Exam for School Entry  
Health Entry Requirements      
This file will provide health information for Kindergartners and First Grade students entering school for the first time. An oral health exam is a new requirement for students in the first grade. Also kindergarten students registering must have an oral health exam.
Immunization Requirements       
The State of California mandates vaccinations for all students attending school. Click on the file to view current requirements.
